I am not a guru.
I have seen the concept of a win-back page used before. I have also been redirected to a page where the publisher asks for feedback as to why I am leaving.
Whether these approaches have been beneficial can only be determined through testing.
Having said this, you want to make the leaving process as simple as possible.
By requiring someone to verify they want to leave, or to type in their email address and click submit, or require the person to complete a survey about why they are leaving, before you let the person go, tends to take on "spammy characteristics." This is something you want to stay away from.
Why? If someone wants to go, just say bye. You want to make the process quick, easy and painless. No harm, no foul.
As to an options approach, the Act does allow for some freedom, stating:
"(B) MORE DETAILED OPTIONS POSSIBLE
The person initiating a commercial electronic mail message may comply with subparagraph (A)(i) by providing the recipient a list or menu from which the recipient may choose the specific types of commercial electronic mail messages the recipient wants to receive or does not want to receive from the sender, if the list or menu includes an option under which the recipient may choose not to receive any commercial electronic mail messages from the sender."(The subparagraph reference is to the statutory obligation to provide an opt-out mechanism.)
